SchoolProgram Text Name of the participant's school, or, if not in school, a program name if applicable.  School names may be typed or selected from a dropdown list.
SchoolCode Text The 8-digit school code. This is filled in automatically when you choose the school from the dropdown list. For private schools, alternative schools or other schools not on the dropdown list, we can add a school code and/or use all zeroes - 00000000.
StartMM Text The placement start date and end date are entered through a dropdown list. StartMM is the start month, and is 2 digits, such as ‘01’, ‘02’, ‘03’, etc., or ‘0’ if missing Required
